Daily Health Screening Update

In order to comply with the NYSDOH, NYSED, and CDC guidelines for the opening of schools, all staff and students will complete a daily health screening.

Whether a child is learning in-person or remotely, all families need to log into the ParentSquare website or download the ParentSquare app. to successfully complete the daily health screening. Students must complete the health screening prior to entering a bus or school building/grounds. Students learning remotely may need to come to the buildings at times so we are asking that families of remote learners also complete the health screening daily.

It is highly recommended that you set your calendar as a daily reminder M-F to complete the survey each morning.

ADDITIONAL INFORMATION ON COMPLETING THE SCREENING 

If a symptom cannot be attributed to any other health condition, stay home, contact your healthcare provider and your school nurse. When parents complete their screening on ParentSquare for the individual student(s), parents will complete a series of questions regarding student symptoms. If a student has one symptom that they are experiencing in the list of questions, the app will let the parent know that they need to stay home and follow up with a physician and school nurse if needed.

If you have not already created an account, Go to ParentSquare online.  Once you have created an account and logged in, there will be an orange bar (The screening form will end each day by 1 PM) that appears on the home screen that says “Submit Daily Health Screening”.  Click that orange icon/box to begin. See attached screenshot as an example for PC or Smartphone application.

Each day, the screening form on ParentSquare must be completed and submitted (“Successfully submitted screening”) prior to entering a school building or getting on morning transportation.
• While completing or submitting the screening form on ParentSquare, if the STOP screen appears, and if a symptom cannot be attributed to any other health condition, stay home, contact your healthcare provider and your school nurse.
